Procedure for change
of Residential Address

Procedure for change
of Applicant Name

Change of Residential Address

Application requesting change of address along with photocopy of registration certificate.
New residence proof through:water or electricity or landline phone bills,ration card,passport.
Aadhar is also required.
Send in application either by registered post or by hand.
No acknowledgement will be sent on recieving such requests.

Change of Name

Application requesting change of Name along with affidavit on Rs. 10 stamp paper.
Affidavit to be notorised and must mention the circumstance and conditions that warrant the necessity of the change of name.
Aadhar is also required.
Fees is Rs.200 Send in application either by registered post or by hand.
No acknowledgement will be sent on recieving such requests.
